Client Rights/Informed Consent Procedures/Confidentiality Procedures

Cuyahoga Valley Mindful Health and Wellness is a licensed counseling clinic with many years of experience in a wide range of counseling disciplines.


We believe that trust and mutual respect form the basis of any healthy patient-client relationship.

We also recognize that every individual is unique and requires their own tailored treatment plan. As a result, we work within a wellness context that avoids generic solutions. We aim to help clients identify what works for them.

Client's Rights

1. The client may ask questions about the therapeutic process and potential outcomes.

2. The client may opt not to undergo specific treatments and techniques proposed by the therapist.

3. The client may choose to discontinue therapy sessions at any time.

4. The client may choose to review any ethical concerns or lack of progress in counseling with any dependently-licensed clinician's supervisor.

5. The therapist may choose to discontinue therapy sessions at any time.

6. On request, the client may review records pertaining to therapy sessions. 7. Within stipulations set out in law, all client records will remain confidential. 8. Information will not be shared with any person, agency, or organization without the prior written consent of the client.

Confidentiality:

The counselling relationship is built on trust between counsellor and client, therefore anything that is shared in session will remain confidential. There are some limits to this, which are:

​

1. If I believe you or another is at risk of imminent harm: where possible I would let you know and discuss it with you, and my supervisor, before taking appropriate action.

2. If a Court Order is received demanding records (Only relevant information subpoenaed will be revealed.) 

3. For the purposes of professional supervision and education. I can ensure confidentiality in face-to-face communication, but cannot ensure that electronic communication by telephone or internet will be confidential.

Please be aware of this if you chose to communicate through e-mail, text, social media, Skype, Zoom or any similar format.
Records and notes from sessions are the property of Cuyahoga Valley Mindful Health & Wellness.

I can supply you a summary of those if requested.

Confidentiality For Parents/Children & Teens:

 Parent(s)/Guardian(s) reserve the right to request session notes and updates in progress throughout the counseling & therapeutic process; however, if you as a patient request for some information to not be disclosed, you and your therapist will process the advantages/disadvantages of revealing information to your parent(s)/guardian(s).



Exceptions to Confidentiality for Children & Teens:

Exceptions where a therapist would need to reveal information to your parent(s)/guardian(s) or other legal authorities include, but are not limited to the following: 

1: Imminent Threats of Harm to Self/Others

2: Court Order

3: Legal request by Parent(s)/Guardian(s). 

4. Abuse or Neglect of the Child/Teen or any Special Populations (Elderly, Disabled, etc.)
